Assertive Community Treatment (ACT Team)

The ACT (Assertive Community Treatment) program brings together a mobile team of F·E·G·S professionals to provide comprehensive treatment, rehabilitation, case management, and support services to individuals with severe mental illness who need a personalized approach to care.

The ACT program provides assistance with:

• Housing – finding safe, affordable housing
• Education & Training opportunities – identifying interests, skills, and supporting participation in school or training
• Daily Life Skills – learning personal care and safety, shopping, using public transportation, and accessing community resources
• Health issues – scheduling medical care, developing a relapse prevention plan, and teaching stress management skills
• Money Management & Entitlements – teaching personal budgeting skills and coordinating government entitlements
• Medication Support & Treatment for Substance Abuse

The ACT Team works closely with consumers to develop a comprehensive, individualized plan that identifies their strengths, preferences, needs, and goals. The Team identifies risk factors and other barriers to success, and monitors the response to treatment, rehabilitation, and support services.
